Dr Johnston graduated with Honours in Obstetrics and Gynaecology from the Royal College of Surgeons in Ireland, Dublin in 1995. After gaining a sound basic training in leading Gynaecological and Obstetric hospitals in Dublin, he moved home to Northern Ireland.

He achieved his membership from the Royal College of Obstetricians and Gynaecologists (MRCOG) at a very early stage in his career. During his further post graduate training he achieved a Diploma in advanced obstetric ultrasound, in addition to recognition in gynaecological ultrasound from the Royal college of Obstetricians and Gynaecologists and Royal College of Radiologists.

Seeing the future of gynaecological surgery for women was minimal access surgery and realising this training was very limited in mainland United Kingdom and Ireland, he travelled to Sydney, Australia to undertake a fellowship in advanced laparoscopic gynaecological surgery. Bringing these advanced surgical skills back to Northern Ireland, he is and remains one of the provinces most established and leading gynaecological surgeons. He was the Chair of the Irish Gynaecological Endoscopic Society 2010-2012.
